Meningitis kort na neurochirurgische ingreep (<=4 dagen)

Broncontrole noodzakelijk: consulteer neurochirurg.

Etiologie

  • Staphylococcus aureus
  • Enterobacteriaceae
  • Pseudomonas aeruginosa

Therapie / dagdosis

Standaard therapie:

cefepim 2 g IV OD, gevolgd door 4 g via CI over 24 uur

Duur: afhankelijk van klinische (en radiologische) evolutie

Opmerkingen

Aanpassen antibiotica in functie van resultaat cultuur.
Eventueel kan amikacine 20 mg intrathecaal elke 24 uur (maximaal 3 dagen) worden toegevoegd.

Commentaar

Indien risicofactoren voor infectie door methicilline resistente Staphylococcus aureus (MRSA), neem contact op met het MIT en overweeg associatie van vancomycine.
