Ernstige gastro-enteritis (+ immuungecompromitteerd)

Ernstig indien 

  • ≥ 6 slappe defecaties/dag
  • aanwezigheid van macroscopisch bloed en/of leukocyten in de faeces
  • koorts ≥ 38,5 °C
  • tenesmus
  • ernstig onderliggend lijden / immuungecompromitteerd

Indien voorafgaande antibiotherapie, alsook bij gebruik protonpomp inhibitoren of cytostatica: denk ook aan Clostridioides difficile !

Opsporen van de verwekker (faecesstaal voor moleculaire diagnostiek, evt in combinatie met cultuur) is absoluut noodzakelijk.

Indien reisanamnese positief: consult MIT.

Etiologie

  • Virussen: Rotavirus, Norovirus, Adenovirus (serotype 40 & 41), Astrovirussen
  • Bacteriën: Salmonella enterica, Campylobacter species, Yersinia enterocolitica, Shigella species, Escherichia coli (EHEC, ...)
  • Parasieten: Entamoeba histolytica (reisanamnese)

 

Therapie / dagdosis

Therapie:

azithromycine 500 mg PO elke 24 uur

Duur therapie: 3 tot 5 dagen

Opmerkingen

Steeds correcte rehydratatie voorzien.

Antibiotherapie is controversieel bij diarree door EHEC (verhoogd risico op HUS) en bij Salmonella (verlengd dragerschap).
