Artritis - Prothese

Therapie met antibiotica wordt indien mogelijk uitgesteld tot na heelkunde (spoeling of revisie).

Absolute indicatie voor diagnostische punctie en/of biopten voor kweek en antibiogram:
- Afname gewrichtsvocht.
- Afname van minimum 3 goede periprosthetische biopten.

Duur antibioticatherapie:
          - Indien na 5 dagen alle bioptkweken en kweek gewrichtsvocht nog negatief zijn, kunnen antibiotica
            gestopt worden.
          - Indien bioptkweken en/of gewrichtsvocht positief worden: gerichte therapie na multidisciplinair overleg.

Consult MIT noodzakelijk.

Etiologie

  • Staphylococcus aureus
  • Coagulase-negatieve stafylokokken
  • Enterobacterales
  • Pseudomonas species
  • Cutibacterium acnes
  • ...

Therapie / dagdosis

Al dan niet empirische antimicrobiële therapie ifv casuïstiek en na overleg met MIT.

Antibiotherapie op basis van kweek en antibiogram en in overleg met MIT.

Duur: te bepalen, afhankelijk van evolutie, in overleg met MIT.
